CareOne at Raritan Bay Medical Center is a Long Term Acute Care Hospital Unit, the first of its kind to open in New Jersey.
What is a Long Term Acute Care Hospital (LTACH)?
Long Term Acute Care Hospital Units are designed to serve Patients with complex medical needs requiring lengthy treatment programs (usually between 25–30 days) in an acute care setting.
The CareOne at Raritan Bay Medical Center LTACH is located on the second floor of Raritan Bay Medical Center. The Unit is certified by the Joint Commission on Accreditation of Health Organizations (JCAHO). It has been extensively refurbished with the same attention to care and detail for which CareOne has become known. It has the capacity to treat 26 acutely ill Patients through an interdisciplinary plan that comprises Board Certified physicians, an excellent nursing staff, a comprehensive social service department, Respiratory and Physical Therapists, Registered Dietician, and 24-hour pharmacy.
Admission Criteria -- Prior to admission, the Patient must be medically stable and all diagnostic workups should be completed with no planned surgical procedures pending. The Patient should also have a prognosis requiring a prolonged stay in an acute care setting with a reasonable expectation for an improvement in the status of his or her medical condition. Additionally:
* The Patient may be ventilator-dependent with fair to good expectation of being weaned
* The Patient may have a new tracheotomy or require ongoing treatment for respiratory failure
* The Patient may have extensive non-healing wounds that may have failed to show improvement or that show a potential for flaps or future surgery
* The Patient may require long-term antibiotic therapy, pain management, or may be receiving TPN/PPN or tube feedings as their sole nutritional source.
* The Patient may require frequent monitoring of pulse oximetry or other critical services such as stat lab work or IV infusions of certain medications.
